I General provisions

  1. This document, hereinafter referred to as the Regulations, defines the rules for conducting postgraduate education for dentists by GRAND ORTHO CENTRUM EDUKACJA Sp. z o. o., based in Warsaw 03-046, ul. Bruszewska 28, hereinafter referred to as the Organizer.

  2. The Organizer informs that the provisions governing the protection of consumer rights do not apply to the mutual obligations of the parties, as the courses are addressed to people professionally related to the subject of the courses.

  3. The regulations include:

    1. a) method of organization and mode of education

    2. b) rules and procedure for recruiting participants

    3. c) rights and obligations of persons participating in education

    4. d) the amount of fees for participation in education

    5. e) final provisions

II Method, mode and place of education

  1. Grand Ortho Education Center Sp. z o. o. is entered in the Register of Entities Providing Postgraduate Education for Doctors and Dentists kept by the District Medical Chamber in Warsaw.

  2. The form and conditions of education, the course program and regulations have been checked and confirmed by the District Medical Chamber in Warsaw.

  3. Postgraduate education of doctors conducted by the Organizer takes the form of a theoretical course with the possibility of practical classes.

  4. The training base is a conference room adapted to the nature of the course, rented for the duration of the training. The Organizer’s registered office does not constitute a training center.

  5. The Organizer reserves the right to change the date and place of the course due to conditions beyond the Organizer’s control, of which course participants will be informed immediately.

III Rules and procedure for recruiting participants

  1. The condition for participation in the course is to pay 100% of the training price. Registration by e-mail, telephone or via the website www.grandortho.pl/kursy does not entitle you to participate in the course.

  2. The number of training participants is limited.

  3. The date of payment of the course fee determines the order of registration.

  4. Each Participant will receive a certificate of completion of the course and an invoice issued on the basis of the data provided by the Participant.

  5. In the event of great interest exceeding organizational conditions, a reserve list is opened (subject to registration). Other people on the reserve list will be informed by the Organizer about any vacant places.

  6. The Organizer reserves the right to cancel the course in the event of too few participants or random events beyond the Organizer’s control. At the same time, he undertakes to immediately inform all participants about this fact.

  7. If the course is canceled, the payment transferred to the Organizer’s account will be fully refunded to the Participant.

  8. In the event of course cancellation, the Organizer does not cover other costs incurred by the Participant, i.e. hotel, travel, etc.

  9. The organizer is not responsible for items left at the training site, during and after the training.

IV Rights and obligations of persons participating in education.

  1. The course is subject to approval by the District Medical Chamber in Warsaw, in accordance with the provisions of the Regulation of the Minister of Health of October 6, 2004. on ways of fulfilling the obligation of professional development of doctors and dentists.

  2. In accordance with the provisions of the Regulation referred to in point IV 1, each Participant will receive educational points for participating in the course.

  3. After completing the course, participants will receive a written certificate of participation and educational points received.

  4. The certificate will include the following information:

    1. a) Name and surname of the participant

    2. b) the participant’s NPWZ

    3. c) course title

    4. d) date and place of the course

    5. e) name and surname of the Scientific Director

    6. f) signature of the Scientific Director

    7. g) name of the organizer

    8. h) podpis organizatora

    9. i) information about the accreditation of the organizer and the course

  5. Video recording during the course is prohibited. Taking photographs without the consent of the lecturer(s) is prohibited.

  6. Images of patients presented in lecture materials are protected by law, taking photos without the express consent of the lecturer is prohibited.

V Fees for participation in education

  1. The course fee amount is provided on the Organizer’s website.

  2. The fee should be paid in full at one time by the deadline specified by the Organizer.

  3. The fee determined by the Organizer should be paid to the account of Grand Ortho Centrum Zdrowia Sp. z o. o. mBank SA Account number: 65 1140 1010 0000 5742 7900 1001

  4. The fee for participation in the training does not include the costs of travel, accommodation and parking for the Participant.

  5. The participant has the right to withdraw from courses on the following terms:

    1. a) More than 30 days before the course Grand Ortho Centrum Zdrowia Sp. z o. o. returns the full payment made to the Organizer’s account, provided that information about the Participant’s resignation is sent to the Organizer’s e-mail address: grandortho@grandortho.pl.

    2. b) More than 14 days and less than 30 days before the Grand Ortho Centrum Edukacji Sp. course. z o. o. retains the right to charge a fee for organizing the course in the amount of 30% of the course price.

    3. c) Cancellation within 14 days of the course will result in no refund of the course fee.

    4. d) In exceptional cases, the Organizer, after individually agreeing with the Participant, may receive the payment

      treated as part or all of the fee for other training scheduled at a later date.

    5. e) Confirmation of participation in the course is tantamount to having to pay for the course, even if you do not arrive

      on the course

    6. f) A Registered Participant may transfer his/her place on the list of participants to another person after prior notice

      informing the Organizer.

    7. g) The participant is obliged to personally participate in classes conducted as part of the course. Presence

      during classes will be confirmed by a signature on the attendance list. Failure to attend any of the meetings, even for higher reasons, does not release the participant from the obligation to pay the full amount for the course.

VI Final Provisions

  1. The content of the Regulations and any changes thereto can be found on the Organizer’s website. Participants will be informed about any changes to the Regulations.

  2. By paying the registration fee and registering for the training, the course participant consents to the processing of his or her personal data to the extent necessary to organize the course, receiving commercial information, processing of personal data for commercial and marketing purposes and the publication of photos taken during the course in the organizer’s marketing materials, websites and Facebook.

  3. Participants have the right to access, correct and delete their personal data.

  4. By registering for the course, the Participant declares knowledge of the Regulations and undertakes to comply with them.
